  • Prenatal Care
  • Physical Examination
  • Social/Family History Intake
  • Ultrasound (Onsite and Referral)
  • Domestic Violence Counseling
  • Health Counseling and Education
  • Immunization Updates
  • Laboratory Services
  • WIC Eligibility

Eligibility: Sliding fee scale with proof of income required. Eligibility is based on gross household income. No patient will be refused services when presenting for care based on lack of documentation, however, each patient will be billed at 100% until proof of income and family size is provided to the agency. The patient will have (14) days to present this documentation in order to change the previous 100% charge to a sliding fee scale. If no documentation is produced in (14) days then the charge stands at 100% for that visit.
